Senior Network Health, LLC can be your alternative to nursing home care. It is a managed, long-term health care plan if you need assistance with day-to-day health maintenance and support activities, but are still able to live in the comfort of your home with family and friends nearby. Nursing professionals with expertise in community-based, long-term care create and coordinate a plan of care that is designed to fit your individual care needs.

• Audiology - hearing exams, hearing aids and batteries
• Dental care
• Durable medical equipment
• Enteral and parenteral supplements
• Home-delivered and congregate meals
• Home health care
• Home health aide
• Medical social services
• Nursing
• Occupational therapy (OT)
• Physical therapy (PT)
• Speech therapy (ST)
• Housekeeping
• Medical and surgical supplies
• Medication management
• Nursing home care
• Nutrition counseling
• Optometry - eye exams and glasses
• Personal care
• Personalized care management and coordination
• Personal Emergency Response Systems (PERS)
• Podiatry - foot care
• Prosthetics and orthotics
• Rehabilitation, (PT, OT, ST) provided in settings other than home

Who can enroll in Senior Network Health, LLC?

You may join Senior Network Health, LLC if you:

• Are a minimum of 18 years of age
• Reside in Oneida or Herkimer County
• Are eligible for Medicaid or willing to pay privately and Senior Network Health, LLC has not reached its private-pay participant enrollment limit
• Are able to remain safely at home
• Are eligible for nursing home level of care
• Will need at least one of the following services and care management for at least 120 days:
  • Nursing service in your home
  • Therapies in your home
  • Home health aide services
  • Personal care services in your home
  • Adult Day Health Care
  • Social day care if used as a substitute for in-home personal care services

When I call 624-HOME, what kind of service can I expect?

A professional staff member will be available to answer your questions. After hours we have staff available to respond to all emergency issues. Our staff is available 24 hours-a-day, 7 days-a-week to answer questions, provide urgent services at home, or assist with hospital discharge needs.

How do I pay for Senior Network Health, LLC?

Services are provided at no cost to Medicaid-eligible members, unless you have a required Medicaid Spend-down. The plan can receive payments from New York State Medicaid. Senior Network Health, LLC is also available to a limited number of private-pay participants.

What is the Medicaid Spend-down Program?

The Medicaid Spend-down Program is a New York State program that allows certain individuals who fit within the federal categories of Medicaid to qualify for Medicaid coverage even though they receive income in excess of eligibility requirements. Spend-down is another term for deductible; it acts like an insurance premium that the you must pay (or incur the cost of) before Medicaid will pay. Click here for more information.
